Job Description UW MEDICINE POST-ACUTE CARE (PAC) has an outstanding opportunity for a PROGRAM OPERATIONS MANAGER, POST-ACUTE CARE WORK SCHEDULE 100% FTE Primarily Remote - Onsite as business needs require DEPARTMENT DESCRIPTION UW Medicine Post-Acute Care (PAC) Program POSITION HIGHLIGHTS Provides operational support for the UW Medicine Post-Acute Care (PAC) Program and assist PAC leadership in executing business and operational strategies for growth, development and evaluation PRIMARY JOB RESPONSIBILITIES Building operational infrastructure for efficient information sharing and deliverables tracking with internal and external stakeholders Works closely with nursing leadership at UW Medicine hospitals and PAC Network partners to evaluate nursing practice, extend standard processes across the continuum Partners with subject matter experts (SMEs) to develop project charters, roadmaps and detailed work plans that enable the PAC Program to achieve defined goals Manages and executes multiple projects according to project plans and timelines; provides management, coordination, direction and support to the PAC team and implements tasks as identified by the project plan Leads the design, creation, validation, analysis and communication of data and dashboards as it relates to PAC priorities, in partnership with finance, analytics and other relevant teams Maintains a professional relationship with stakeholders at all UW Medicine entities at all levels, and PAC Network partners, including Skilled Nursing Facilities, Home Health Agencies, Hospice Services, Adult Day Health programs and Home Care Agencies Manages marketing activities for PAC (e.g., updates to public facing website, updates to flyers, development of SharePoint intranet site) REQUIREMENTS Bachelor's degree Two to three years of working in a large health system with multiple EHRs, medical staff models, and an array of clinical services Demonstrated experience working on quality and process improvement projects with cross-functional teams, including data collection, analysis and reporting Superior organizational skills with demonstrated ability to multi-task and manage multiple projects at time, while maintaining clear attention to detail Ability to work independently with minimal supervision and manage multiple priorities Strong collaborative, proactive approach to job functions; anticipates problems and seeks solutions Proven record of well-developed analytical, communication, interpersonal and problem solving skills, including tolerance for ambiguity Significant and demonstrated proficiency in Microsoft Office applications and ability to learn new computer/software skills quickly Customer orientation - establishes and maintains long-term relationships, building trust and respect by consistently meeting and exceeding expectations with internal and external customers, vendors and stakeholders Ability to understand and interpret regulatory standards, clinical guidelines, national quality metrics and health services research, including post-acute care Displays professional, ethical conduct and maintains confidentiality of information Equivalent experience may substitute for degree requirement ABOUT UW MEDICINE – WHERE YOUR IMPACT GOES FURTHER UW Medicine is Washington’s only health system that includes a top-rated medical school and an internationally recognized research center.
